DATA CONTROLLER:
The company BIOTEC S.r.l., with registered office in Via Industria 53, - 36031 - Povolaro di Dueville (VI) Italy, VAT No. 02687460242 - email: privacy@btk.dental, represented by its delegate, Mr LUCA FINCO - Tax Code FNCLCU72B17Z133J, as data controller, in accordance with and by virtue of Art. 13 of Regulation (EU) 679/2016, hereby informs the data subjects of the processing purposes and methods of the personal data collected, their scope of application and dissemination, as well as the nature of their provision.

PROCESSING PURPOSE:
The personal data, subject to processing, in compliance with and in respect of the rules on personal data processing and protection, are used:
•	to fulfil purposes instrumental and functional to carrying out the pre-contractual and/or contractual relationship in place, which involves obtaining new clients/partners, managing orders/jobs for goods or performances of services, including professional and qualified services, and also procedures connected to those activities, such as archiving, invoicing, processing and registration.
•	to fulfil legal obligations (for example, accounting and tax fulfilments).

NATURE OF PROVISION:
The provision of data is mandatory for the fulfilments required and sanctioned by law. For anything else, the provision of data is optional, but necessary. 
Any refusal by the data subject shall lead to the impossibility for the Controller to establish the relationship and to execute it precisely. The data collected will not be disclosed or disseminated. The data may be communicated to third parties, other than the Controller, by internal representatives and managers external to the company and by employees assigned to perform processing, who are duly identified and appointed in accordance with the provisions of the regulation, in order to fulfil legal requirements as well as, where necessary, to pursue the indicated purposes. Within the limits of the same, the data may be communicated to third party bodies and companies, such as legal and industry advisors, credit institutions, insurance companies, credit recovery companies and contractual consultancy firms, shipping agents, third party supply companies and technical and IT support firms, and companies operating in the IT sector. In any case, third parties may only process the data according to fairness and in respect of existing laws.

DATA PROCESSING FOR CUSTOM-MADE MEDICAL DEVICES:
If you request the creation of custom-made Devices, you declare, in accordance with Regulation (EU) 2016/679 and Italian Legislative Decree no. 196/2003, that you have obtained from the patient authorisation to process his/her personal data to be sent to Biotec S.r.l. for the purpose of creating the requested custom-made Device. 
You also declare that the patient is aware that those personal data will be stored for 15 years at Biotec S.r.l. to guarantee the traceability of the product, as required by law. The personal data, of which Biotec S.r.l. is in possession, will be used for the sole purpose of creating the requested custom-made Device; those data will not be used by Biotec S.r.l. for statistical purposes or marketing activities. 
Biotec S.r.l. recommends, where possible, that you do not send files containing the patient's personal data, but, instead, make those data anonymous using appropriate coding.
